The area between the entrance of the vagina and the anus is called the ‘Perineum’. Aesthetic and functional correction of tears in the perineum after vaginal delivery, suture scars due to episiotomy incision repair or skin folds extending from the inner lips to the perineum is called ‘Perineoplasty’. It is often performed simultaneously with vaginal tightening surgeries.
“In the perineum, painful sexual intercourse may be encountered due to opening of the sutures after vaginal delivery or poor wound healing. In addition to the  damage due to incisions and tears after vaginal delivery, there may also be damage due to traumas such as bicycle and car accidents. In the perineum, the muscles surrounding the vagina and the anus are combined. Perineal damage can also affect these muscles. There may be complaints of gas and stool incontinence.

‘Perineoplasty’ is an operation that provides both aesthetic and functional benefits. With this surgery, the external genital area gains a more aesthetic appearance. The distance between the entrance of the vagina and the anus is lengthened, the excess vaginal opening is closed, the tightness at the entrance of the vagina is increased, the angle of the entrance of the vagina is corrected, and if there is a need for muscle repair around the anus, it is also performed. It is aimed to increase the clitoris and G-spot stimulation during intercourse and the problem of painful sexual intercourse can be eliminated.

What is perineoplasty? 

Perineoplasty is a surgery in which both aesthetic and functional repair of the perineum is performed by repairing congenital defects of the perineum, and damage due to trauma to the perineum, including vaginal delivery, or removal of the skin folds of the inner lips extending to the perineum. It is often performed simultaneously with vaginal tightening surgeries.

What are the benefits of perineoplasty?

Under 18 years of age, those who want to have a vaginal birth afterwards, pregnant women, breastfeeding mothers, those with psychiatric illnesses and those with genital infections are not eligible for this operation.

What are the risks of perineoplasty? 

There may be risks such as bleeding, infection, oedema, early opening of stitches. Proper surgical technique and postoperative care minimise these risks.

What should I do before the operation?

Hair removal of the external genital area should be done.

Step by step operation process:

Anaesthesia: General anaesthesia is applied.

Surgical procedure: Damaged tissues over the tears and stitch scars of the deliveries are removed. The muscle tissues underlying the skin are also repaired. Absorbable very thin suture materials are used for repair.

What should I expect after perineoplasty? 

Pain, swelling, slight bleeding in the form of leakage for a few days and itching in the suture area may be observed. Complaints will decrease over time.

What should I pay attention to after perineoplasty? 

Wound care should be done as recommended by your physician. The first 3 days should be spent resting at home.  The recommended treatment should be followed. Anti-constipation medication should be used for a week. You should pay attention to the cleanliness of the genital area and pay attention to your diet. Also, let us remind you that smoking negatively affects wound healing by impairing oxygenation in tissues. After one week, it is recommended to perform Kegel exercises. These exercises strengthen the pelvic muscles and make the results of the operation more favourable.

How long should I rest after perineoplasty?

Home rest is recommended for one week after perineoplasty.

When can I start working after perineoplasty?

You can start working on average 1 week after perineoplasty.

How long is the recovery period after perineoplasty? 

The recovery period after perineoplasty is 4-6 weeks on average. You can have intercourse after 6 weeks.

Damage to the perineum can occur as a result of traumas such as tears in the perineum after vaginal birth, assisted delivery, episiotomy, bicycle or car accidents.

Painful sexual intercourse may occur as a result of poor wound healing of birth tears and stitches in the perineum. With perineoplasty, this scar tissue can be removed and the complaint of painful intercourse can be eliminated. The external genital area gains an aesthetic appearance. Tightening the vaginal entrance and correcting the vaginal entrance angle can have a positive effect on sexual pleasure.

There is no excessive pain after perineoplasty. However, there may be moderate pain due to swelling.

Ice application is continued intermittently for the first 3 days after leaving home. Ice application is important for the resolution of bleeding, pain and swelling. It is generally recommended to apply antibiotic creams or solutions regularly for 7-10 days. In addition, you should pay attention to your hygiene and clean your perineum after going to the toilet always from front to back. There is no harm in washing the wound with water or soap.

To accelerate wound healing after perineoplasty, you should first of all follow your physician’s recommendations. You should take your antibiotics and painkillers as directed. At home, apply a cold pack of ice on your underwear for 20 minutes. Remove the cold pack for 20 minutes and then apply it again. Elevate your bottom with pillows when lying down. Keep showers short, do not bathe in the tub, and gently dry the area. After going to the toilet, wipe the area dry; do not rub it. Also, wear loose clothing to avoid irritating the area. Do not have sexual intercourse or use tampons for 4-6 weeks.

Use only warm water or recommended hygienic solutions to wash your wound after perineoplasty; do not use soap. Gently dry the area before and after urination, but do not wipe; do not rub the surgical site. Your physician may ask you to take medication or follow a diet to prevent constipation, which can stretch your stitches or open the surgical wound site.

You should inform your doctor immediately if you develop excessive swelling or worsening pain, increased bleeding; if you see clumps of blood, colourless pus or discharge; if you experience severe pain when urinating or defecating; if you have fever or if there is opening of the surgical site.

You can start working approximately one week after perineoplasty. Sports such as heavy exercise, swimming, horse riding, cycling, yoga, pilates, running are only allowed one month after the operation.

You can take a shower 4 days after perineoplasty.

You can have sexual intercourse approximately 5 weeks after perineoplasty.

It is not recommended for patients who will undergo perineoplasty to give birth vaginally afterwards.

Perineoplasty is not performed during menstruation. Preferably immediately after the end of menstruation, but it can be performed in any menstrual period without bleeding.

There is no harm in laser epilation six weeks after perineoplasty.
